Update on Ulcer Debridement Coding and Documentation

The documentation requirements associated with ulcer debridement are extensive.  Providers who perform ulcer debridement must be acutely aware of the long list of items that must be documented when this service is performed.  In this session, the sources of these requirements will be shared so providers know where to go to find the requirements they need to know.  The list of documentation requirements will also be shared and explained.  Additionally, time will be devoted to CPT® code selection for ulcer debridement, including how to know which code to use based on the debridement performed and what needs to be documented to support submission of the CPT® code that was submitted.
